Automation matures by knowing when it must not continue alone, rather than by removing every person. Escalation boundaries turn uncertainty into a controlled process and protect customers, teams and data.
Map irreversible actions
Identify payments, deletions, public responses, permission changes and decisions affecting people. These actions require stricter thresholds or mandatory approval.
Define observable stopping conditions
Use missing required fields, conflicting sources, prohibited categories or scores below a defined threshold. “If you are unsure” is insufficient without an observable signal.
Design the exception queue
Every escalation needs an owner, priority, complete context and response time. If cases merely accumulate, a safety measure becomes a hidden delay.
Do not let the model fill critical gaps
When essential information is missing, the right behaviour is to ask or pause. A plausible answer must not be presented as fact just to keep the workflow moving.
Learn from each escalation
Record the cause, human decision and final outcome. Recurring exceptions reveal whether better inputs, a new rule or narrower scope are needed.
